Client Info We prefer to have clients print out the registration form BEFORE coming to their first appointment or clients can fax the form to the office at 718-331-7099. Please arrive early to fill out an information sheet. Be sure to bring the following with you:
We request payment at the time of service and we accept all major credit cards. We will always talk with you about the cost of treatment before undertaking a major procedure. We realize that the cost of treatment can be high and that people often face difficult decisions about whether they can afford needed treatment. Whenever possible, we will try to arrange a payment plan or refer you to subsidized services. Whenever your pet is admitted to the hospital you will first be given an estimate our services. We gladly accept all major credit and debit cards as well as personal checks. An alternative option is to apply for a credit card specifically for your pet.
